Hospice Health Care Network provides compassionate care and support for the terminally ill and their loved ones. Our mission is to coordinate appropriate care and services making the journey during the last stages of illness and during bereavement, as comfortable as possible providing the physical, spiritual and emotional support needed.

Hospice is not a place – rather, it is a special concept of care designed to provide comfort and support to patients and their families when a life-limiting illness no longer responds to cure-oriented treatments. Hospice care neither prolongs life or hastens death. Hospice addresses all symptoms of a disease, with a special emphasis on controlling a patient's pain and discomfort. Hospice also provides a variety of bereavement and counseling services to families before and after a patient' death.

Hospice Health Care Network focuses on enhancing a patient's comfort and overall quality of life. Each patient is served by a core interdisciplinary team of professionals and hospice volunteers who are guided by the wishes and decisions made with the patient, their family and physicians.

To qualify to receive hospice care, the patient must:

  • Be diagnosed with a terminal illness
  • Have an order from a physician
  • Have a life expectancy of six months or less
  • Have a desire for palliative care rather than curative, aggressive treatment.
  • Have a caregiver who is available and willing to participate in the patient's care.
